View Required Documents
- Duly completed draft application form
- Two passport size colored picture (white background)
- Original passport (6 months valid)
- Old passport issued within the last 10 years
- Original personal bank certificate (must include account type, current balance, account opening date, 6 months average daily balance)
- Original bank statement or certified true copy of passbook for the last 6 months
- Original PSA birth and marriage certificate
SPECIAL REQUIREMENTS
FOR EMPLOYEES
- Original employment certificate with leave of absence (must include applicant’s position, date hired, salary, date of return to work)
- Copy of latest individual tax return
- Copy of PRC/IBP ID (if professional)
- Copy of company ID
FOR BUSINESS
- Copy of business registration (DTI or SEC)
- Copy of business permit or mayor’s permit
- Copy of income tax return
- Copy of financial statements
FOR STUDENT
- Original school certificate
- Copy of school ID
- Leave of absence from school (if traveling while classes are ongoing)
- Affidavit of support and consent form (signed by both parents)
- Scanned copy of the parent’s full bio page data of the passport
 FOR RETIREE
- Copy of senior citizen card or retirement documents
FOR FOREIGN NATIONALS
- Copy of alien certificate of registration (ACR)
- Copy of international card residence (ICR)
- Re-entry permit
SPECIAL NOTES
- Turkey visa sticker application is applicable for Philippine passport holders who have no valid
US visa or Schengen visa.
- Additional requirements may be required by the embassy without prior notice.
- Turkey visa processing if applicable for those availing tour packages with us.
