| Copy of your passport including the photo page of your passport and any pages with amendments, endorsements, visas and entry / exit stamps. |
| One (1) passport-sized photo (head and shoulders only against a plain background). |
| Birth certificate issued by the Philippine Statistics Authority (PSA) |
| Bank certificate showing the average daily balance for the last three (3) months |
| Bank statement/s showing account transactions for the last three (3) months. |
