- Duly accomplished and signed visa application form - IMM5257 (must be fully completed, dated and signed. You must answer every question. If no applicable, write N/A)
- Duly accomplished family information form - IMM 5707
- Duly accomplished document checklist - IMM 5484
- Two passport sized colored photos (White background)
- Original valid passport (6 months valid) and old passport within 10 years
- Original personal bank certificate (must include account type, current balance, account opening date, 6 months average daily balance)
- Any additional relevant documentation (last 2 pay slips, property titles, proof of pension, investments, etc.)
- Copy of recent personal income tax return
- Hotel booking confirmation
- Round Trip airline ticket reservation
- Detailed itinerary during your trip

FOR EMPLOYEES
- Original employment certificate (must include applicant’s position, date hired, compensation, office address, landline numbers and email address
- Copy of PRC /IBP ID (if professional)
- Copy of company ID
FOR BUSINESS
- Copy of business registration from DTI or SEC
- Copy of business permit and mayor’s permit
- Copy of PSA marriage contract (If traveling with spouse)
FOR STUDENT
- Original school certification
- Copy of school ID
- Copy of PSA birth certificate (If traveling with immediate family)
- Copy of parent’s documents, original bank certificate, original COE or business documents
- Affidavit of consent from the non-accompanying parent(s) or legal guardian (if traveling with only one parent or without parents)
FOR RETIRE
- Copy of senior citizen card or retirement documents
- Copy of PSA marriage contract (If traveling with spouse)
FOR FOREIGN NATIONALS
- Copy of i-card
IF THE COMPANY WILL SHOULDER THE TRIP
- Original employment certificate for sponsorship
- Copy of business registration from DTI or SEC
