3.One (1) recent colored 45mm x 35mm size photo with white background (Printed photo, taken within the last 3 months and has not been previously submitted to the same embassy where you are currently applying)

10.Immigration Card (ICARD) issued by Bureau of Immigration (Applicable for non-Philippine passport holders Printed copy should be front and back page) (Printed Copy)
11.Guarantee letter (Original) To be completed by the sponsor and to submit supporting papers such as: a. Requirements given in #4, #5 & #6 b. Proof of relationship given in #8 & #9 c. Proof of communications (i.e., photos and/or letter exchanges), in the absence of item letter “b”
12.Multiple Entry Request Form (Original) *For visa applicants requesting for multiple entry, the following criteria should be met: a. Frequent traveler to Japan b. Regular employee by one of the following enterprises and his/her spouse or child c. A cultural or intellectual figures and his/her spouse of child d. A spouse/child of the Japanese in the Philippines with long-term visa excluding tourist visa Wet / ink signature is required
NOTE:
1. For visa applicants with remarks of late registration on their PSA birth certificate, need to submit the original documents such as: Baptismal certificate / Elementary or High School Form 1-37
2. For visa applicants with No Record with PSA, need to secure a Certificate of No Record Found issued by the Local Civil Registrar or a live birth certificate issued by the National Archives.
3. For visa applicants with unreadable details on their PSA birth certificate and/or PSA issued marriage contract, need to submit a certified true copy of their birth and/or marriage issued by the Local Civil Registrar.
4. For visa applicants holding professional licenses (i.e., lawyers, engineers, medical professionals), they have the option to submit a printed copy of IBP ID or PRC ID.
5. We will only accept visa applications with complete documents.

7. Once the visa application is filed, your passport will be in the possession of the Embassy, whether locally or internationally. Please ensure that you have no planned travel immediately before, during and immediately after processing.
